Severe hair loss in 6 months

I lose at least three hundred -four hundred strands every day.And it comes out in piles. I used to have such thick hair, and now my ponytail is very thin. I can literally grab my hair and 5 strands will come out at once from the root.

what can I do or what can I tell my doctor to prescribe me for this? i'm literally going bald😔
